This person will work with all heart rhythm device patients, which primarily includes patients with pacemakers and cardiac defibrillators (ICDs).  The core responsibility will be to ensure that cardiac rhythm device patients have a consistent and safe experience with Lurie Children’s across all care environments (inpatient and outpatient).  The role is mostly outpatient, but some inpatient and procedural coordination is involved.  Most patients will be infants and children, but the nurse will also develop relationships with patients in our Adult Congenital Heart Disease clinic. This person will staff multiple clinic locations including Clark/Deming, Lurie main campus, and Skokie. The outpatient role may grow to include in-person requirements in other clinics, depending on patient volumes. Due to our evolving relationship with Rush Hospital, coordination of care between patients and providers at Rush and Lurie will be expected. 5 years experience is preferred

The Infectious Diseases division at Ann & Robert H. Lurie Children's Hospital is looking to add a nursing team member. This position will help support inpatient and outpatient activities in the division, as well as participate in patient education and quality improvement activities across the division.   As a member of the Transplant ID team, you will serve as a point of contact for our patients and their families, in partnership with the ID Transplant APP. You will follow patients as the primary RN serving to establish strong, compassionate relationships, while managing the day to day clinical care provisions. Responsibilities include, but are not limited to, frequent collaboration with ID physicians, coordination of appointments, ongoing documentation of ID interventions in EPIC, clinic support including vaccine administration & required clinic follow-up needs, patient & family education, lab surveillance, and will ensure coordination and cooperative relationships among health care teams by communicating information in an efficient and timely manner.   In support of the outpatient ID clinic team, you will be the primary contact for our provider at the Central DuPage Hospital. While still being based in downtown, you would be responsible for pre- and post-clinic scheduling and support. You will also be working in conjunction with our existing nurse to create an overlapping support system to allow for cross coverage of tasks during vacation, illness or other times of need.   Along with ID MD/APP guidance, you will participate with the coordination of care for patients and their families with congenital CMV and various fungal infections that require prolonged antifungal therapy with responsibilities that include but not limited to: primary contact for clinical management, lab surveillance, on-going support and continued education throughout the treatment course which includes documentation of ID interventions in EPIC.   We are also in a state of growth and ongoing practice improvement. You will participate in managing and planning for growth of clinical operations, and be expected to design creative solutions to existing challenges on both the inpatient and outpatient settings. We are looking for a motivated RN partner who has pediatric nursing experience, and is an excellent communicator.
