    General Summary of Position Responsibility:

    The Plastic Surgery Clinic Nurse Coordinator, under the general supervision of the Plastic Surgeon, coordinates services with the multi-disciplinary team to provide care to the patients in need of Plastic Surgery. The Nurse coordinator promotes and facilitates optimal care and works with a diverse group of individuals including families, physicians, nurses, donors, Foundation Staff, Institutional Review Board, Children’s Hospital of Chicago Research Center’s staff, Information Management and other hospital staff through clinical, administrative, professional, educational, and quality improvement activities. The position actively supports the Hospital's and the department's Continuous Quality improvement and Customer Service goals through modeling of appropriate behavior and by providing a work environment conducive to attaining these goals.


    Specific Duties and Responsibilities:

    1. Schedules all clinic visits, procedures and special equipment needs.
    2. Coordinates the scheduling with other ancillary departments i.e. Medical Imaging and laboratory.
    3. Coordinates the specific program activities which pertain to the Cleft-lip and Palate, Craniofacial and Plastic Surgery patient and include clinical (ambulatory and inpatient).
    4. Transitions the patient from the Inpatient to the outpatient. Setting Education (both community based and professional education).
    5.  Provide recommendations and assistance to families and physician both within and outside of the hospital Establish and maintain rapport among a group of diverse health care providers.
    6. Maintain an effective working relationship with these individuals. Provide methods of communication with patients, families and the Craniofacial Team.
    7. Coordinates and facilitates the craniofacial team meetings. Provides education to new staff, patients and families in both the inpatient and outpatient settings.
    8. Development and management of a recall database to follow-up patients with missed clinic appointments.
    9. Participates in the development, implementation and ongoing assessment of policies, procedures, and guidelines that define the care of the patient with a Craniofacial Deformity or CL and Palate.
    10. Establishes and maintains rapport among a group of diverse health care providers, as well as maintaining an effective working relationship with these individuals.
    11. Acts as resource to hospital personnel with regard to care and coordination of services for the Craniofacial, CL and Palate and Plastic Surgery Patients.
    12. Collaborates with Clinical Educators to meet the ongoing education needs of the nursing staff. Collaborates with the Program Director and other team members in the planning of educational programs.
    13. Develops or individualizes teaching plans/protocols to meet patient and family learning needs. Identifies patient preferences and needs including motivation to learn. Evaluates and documents changes to the plan whether that be in the Electronic Medical Record or in the paper chart.
    14. Other duties as assigned


    Knowledge, Skills and Abilities:

    1. A BSN degree from an accredited school of nursing and current licensure as a registered professional nurse in the State of Illinois required.

    2. Master’s prepared from an accredited nursing program preferred.

    3. Candidate must have 3-4 years of pediatric nursing.

    4. A minimum of 1-2 years in Neuroscience preferred.

    5. Experience working in partnership with health care professionals and support staff in a clinical setting is necessary. 6. Must demonstrates strong organizational skills, the ability to problem-solve and support teamwork in multidisciplinary setting.

    7. Computer skills required, especially word processing, Excel and Access.

    8. This candidate must demonstrate excellent written and verbal communication skills.
