The Adult Congenital Heart Disease (ACHD) Nurse Coordinator coordinates services with multi-disciplinary teams to provide care to the adult congenital heart disease population. The Nurse Coordinator promotes and facilitates optimal care and works with a diverse group of individuals including families, physicians, nurses and other hospital staff through clinical, administrative, professional and educational activities. The position actively supports the Hospital's and the department's Continuous Quality improvement and Customer Service goals through modeling of appropriate behavior and by providing a work environment conducive to attaining these goals.


  • Coordination and scheduling of clinic visits, testing and specific needs.
  • Coordinates scheduling with other departments as needed, i.e. medical imaging.
  • Maintains communication with patient regarding lab results, Holter monitor results and recommended medication changes.
  • Triage patients calling with symptoms. Provide recommendations to nearest ED or prompt evaluation in clinic setting.
  • Responds to patient’s messages and phone calls within a timely manner.
  • Manage insurance prior authorizations and pre-approvals for medications, testing, and procedures.
  • Provides work, school excusals. Provides dental, surgical, and work clearances.
  • Participates in team meetings and community functions as needed.
  • Educates patient through transition from pediatric cardiology care to adult cardiology care. Provides education to patients and families in outpatient setting.
  • Manages clinic preparation ensuring correct testing is ordered, requested testing is complete with records and follow-up timing is appropriate.
  • Provide recommendations and assistance to families and physicians both within and outside of the institution.
  • Participates in the development, implementation and ongoing assessment of policies, procedures, and guidelines that define the care of adults with congenital heart disease.
  • Establishes and maintains rapport, promoting collaborative communication, among a group of diverse health care providers, as well as maintaining an effective working relationship with these individuals.
  • Acts as resource to hospital personnel with regard to care and coordination of services for patients.
  • Travels with physician to outreach clinic sites to ensure clinic workflow and other responsibilities as needed.
  • Collaborates with Clinical Educators to meet the ongoing education needs of the nursing staff, as well as assisting with the education of new staff. Collaborates with outside hospital nursing staff to provide education on ACHD and importance of continued care.
  • Maintains current knowledge of care of the ACHD patient via literature and educational conferences and offerings.
  • Other duties as assigned.


  • A BSN from an accredited school of nursing and current licensure as a registered professional nurse in the State of Illinois required. Experience working in partnership with health care professionals and support staff in a clinical setting is necessary.
  • Must demonstrates strong organizational skills, the ability to problem-solve and support teamwork in multidisciplinary setting.
  • Computer skills required.
  • This candidate must demonstrate excellent written and verbal communication skills.
